Individual Therapy (53-60 min) $175

*Charge for No-Show for appointment is $50

*Charge for late cancelation (less than 24 hour notice) for appointment is $50

*No show and late cancelation fees are the client's responsibility. Insurance will not cover these charges. 

*Bills are due within 20 days of the statement date. Past due bills are charged a 5% late fee. 

Insurance

Please contact your health insurance to ask if you have outpatient mental health coverage with your insurance plan. You may want to ask about specifics including deductible, copay, if there is a limit to your number of visits, if a physician referral is required, if the provider (Tara Krueger) is included in their network, and if prior authorization is needed. Healthcare plans vary, so it is important to know what is covered by your health insurance.  I am currently accepting the following medical insurances:

Payment Information

Payment is due at the time of service and can be made by cash, check, health savings (HSA), or credit card. I use TherapyNotes™ for electronic billing to process credit cards (which is HIPAA compliant).

I will file the claim with your insurance company. You are responsible for the co-pay, if one is required. If you have a deductible, you may be required to pay for the entire session if your deductible has not been met.  Please contact your insurance company in advance so you will know your financial responsibility prior to starting therapy. If you chose to use your insurance, please bring your card with you to your first appointment; a copy of your card will be kept in your records.

If you do not wish to bill insurance for your services, you can choose to be private pay, in which you pay the full rate for services on your own.
